Intake Forms Our administrator will send you a variety of online forms to complete prior to scheduling the intake assessment. 2. Assessment Sessions During the intake, your therapist will assess your needs, collect your history, and help you articulate goals for therapy. 3. Therapy Sessions A block of sessions will be scheduled to initiate the therapy plan discussed with your therapist. The number and frequency of sessions will be decided together based on the therapy plan and your goals. 4. Ongoing Therapy as Needed As you progress in therapy, the therapy goals and schedule will evolve to ensure that you have the support you need until you are ready to terminate.
